Community Resource Guide

At Hale County Hospital, our mission extends beyond the care provided within our facilities. We are dedicated to delivering exceptional healthcare and ensuring individuals have access to resources that support their well-being long after their visit.

To further this goal, we maintain a regularly updated list of services available to residents of Hale and Perry counties, including assistance with healthcare, housing, food, and education. These resources are designed to help individuals and families overcome challenges and build healthier lives.

  • West Alabama Public Transportation (WAPT)

    • Provides transportation for citizens of Bibb, Choctaw, Dallas, Greene, Hale, Lowndes, Marengo, Perry and Sumter counties.

    • To schedule transportation services, call (334) 289-5789.

    • WAPT is covered by Medicaid or can be paid out of pocket (cash). Not covered by Medicare.

    Rebirth Transportation

    • Provides non-emergency medical transportation in the West Alabama area for Hale, Perry, Tuscaloosa, and Marengo County residents.

    • To schedule transportation services, call (334) 507-2410.

    • Private pay and Medicaid patients are accepted.

    Kid One Transport System

    • Provides children (up to age 19) and expectant mothers to necessary health care services.

    • Cannot pick up in Hale County, but will transport from Greene and Tuscaloosa County if you can meet them there.

      For Medicaid approval, call 1-800-362-1504 within two weeks of your appointment. If approved, you will receive an approval number that must be provided to the transportation agency.

  • The Area Agency on Aging of West Alabama (AAAWA) offers a wide range of services to support older adults, individuals with disabilities, and their caregivers. These services aim to enhance quality of life, promote independence, and provide access to essential resources.

    Core Services

    • Medicaid Waiver Program provides in-home services for individuals receiving Medicaid benefits who are at risk of nursing home placement.

    • Alabama Cares is a program that provides information, assistance, education, respite and supplemental services to caregivers.

    • Senior Activity Centers: AAAWA supports 22 Senior Activity Centers. These centers provide a central location for socialization, education, recreation and health promotion to individuals age 60 and above.

    • Counseling and Supportive Services about Medicare management, prescription drug assistance, health promotions, and legal assistance.

  • Project Horseshoe Farm (HSF) is a nonprofit organization dedicated to strengthening communities and supporting vulnerable populations, including older adults, adults with mental illness, and youth. With locations in Greensboro and Marion, HSF provides a variety of programs aimed at improving health, education, and quality of life.

    Core Services

    • Community Center Program: HSF operates free programs three times a week at welcoming community centers in Greensboro and Marion. Adults can stay active, connect with others, and pursue health goals through activities like exercise, crafts, gardening, music, and wellness clinics. Fresh produce and easy recipes are also available

    • Health Partners Program provides caring, non-medical support to individuals who may feel isolated or need assistance managing their daily lives. Horseshoe Farm Fellows build trusted relationships through home visits, walks, and personalized help with tasks like remembering appointments, navigating social services, and healthy living. Participants gain companionship, guidance, and practical support to improve their health and well-being.

    • After-School Program: HSF operates a free after-school program three times a week at the Greensboro community center for students K-12.

  • The Boys and Girls Club of West Alabama offers programs and services designed to empower youth and support their academic, emotional, and social development.

    Core Services

    • Academic Support: Tutoring, homework assistance, and STEM-focused activities to enhance learning and achievement.

    • Leadership and Life Skills: Programs that promote character development, leadership, and positive decision-making.
